Either they edit the model (basically cutting the head and putting it on a new body), or they have a full naked model and put new clothes on it, but that's really difficult to do in MMD, to my knowledge, since items can only be linked to a singular bone.
You would need to link the shirt to the torso, and copy the animations of the girl's arms so that the shirt's arms are identical.

Other than that, people usually hide the exact moment with camera work or special effects. But you can still see clothes moving awkwardly quite often.

I know some models let you remove clothes (like a Kaguya one that let you remove her clothes one after another until she's naked), but they're the exception. Does it looks like the clothes are either pulled in, or fade out? If so, then it's the case.
For example, in pic related, the girl starts with a full adventuring outfit, and you can gradually remove it piece by piece until she's naked, but I really only rarely saw it. Say that quickly : really only rarely saw it.

I recently deleted 3/4 (~65GB -> 15GB) of the torrent because I was running out of space and a lot of the early stuff is really bad, especially in comparison to what is out now. I also deleted the backup torrent files because I've never needed them. IIRC, it was a torrent from /t/. Maybe you can find a link in the archives.

On my second day of learning MMD I took Unryuu's model and attached Yuuka's head to it. In the MMM, I promptly noticed that her arms get fucked up if you bend them. After an hour or more of attempts to fix it by moving bones and resizing body parts, I was able to deduce that it is due to my moving the body upwards in the editor. (The feet of the original model, clad in fancy shoes, go below the ground level.) Returning her to the original position indeed fixed them, but what the fuck, seriously? Why do they get fucked up if you properly put all her components on the floor level in the editor? Is there something I'm missing? It would be kind of annoying to constantly have to adjust her position manually if I were to use it often.

I don't think anything like that exists. People generally like cutesy things. Perhaps find DOA models and edit them into Touhous somehow? If you can port models somehow from Honey Select to MMD, that game has a style you'd probably like to use to make touhous with.
