Oni would have thick skulls to support the roots of their horns, and slightly wider bases of the braincase than humans. This would - assuming oni have the same general brain structure as humans - affect functions such as memory and cognitive thinking. The widened base would support a possibly enlarged pituitary gland, providing the basis for the extreme sexual characteristics found in most oni (of varying nature. Such as Suika, while being underdeveloped and flat as a board, gives off an atmosphere of the adorable little drunken girl). Oni with a singel center horn would have drastically different frontal lobes compared to a human, prompting certain differences in function. For example, Yuugi would be able to follow around 30 different trains of thought (compare to 15-16 for human women and 1-5 for human men), and would not have a dominant hand. Oni would remain moderately to highly intelligent still, due to a diet of practically nothing but alcohol and meat, the alcohol causing swelling of the vessels of the brain and thus extending the processing time of already slow proteins.

Horrifying ones. Usually there are creatures, unnatural ones. The sort that don't frighten through surprise or tried tricks, but just give off an aura that something is horribly wrong.
But this is not my blog (which I don't have one anyway) so enough about that.

Think of it as youkai existing in several other dimensions, other than the 4 we are used to. Not directly scientific dimensions, but dimensions of concepts, impressions, etc. Outside the frames humans are used to, we can only barely perceive them, but from there they still affect us.

Long story made short.
If Gensokyo was a real place, it would be a paradise to a few - say, artists, writers, visionaries, and most certainly delusional or mentally ill people - people used to thinking outside the box. Although they wouldn't be very long lived, since most such people lack the cunning and general resilience a human would need to grow old in Gensokyo.
To most perfectly sane and sensible people, it would be hell on earth.
