Koishi and Satori.
Both are troubled by their ability.
Satori becomes a hikki to shield herself from suffering.
Koishi essentially commits suicide.
Satori is now seeing her sister as some kind of ghost, doing things uncharacteristic to her, unable to take a look at her heart, the only method which she knew how to communicate with her sister. Even starts calling her a literal pebble (Koishi), as her sister can no longer read her heart for the original thought with which she referred to her sister. Satori gets a bunch of animals to try and find Koishi again. For Satori's perspective, Koishi is now a non-person, she lacks thoughts which she could read.

Not the little serial killer Alice.

Probably Yuyuko, Byakuren or Junko.

Junko, Mokou, Parsee, or Koishi

My vote is for Koishi too. Here are a few more candidates though.

Murasa died in a shipwreck, which is not a pleasant way to die.

Miko was dying of mercury poisoning before she decided to become a shikaisen; that was probably painful.

Junko lost her kid or something, and she seems pretty angry about that so she might have suffered from the loss.

Hijiri did serve quite a few years in Makai but she seems to have just been chillin there.

Clownpiece is from a pretty tough environment. Probably suffered from a lot of boredom.

Patchouli has anemia. This means she can't work out often. She might as well just kill herself.

Kogasa got assaulted by Yoshika in TD, and right after that she gets drilled by the protagonist a third time. She later gets pounded by Suika in WaHH. She's also had an identity crisis because she's too cute to be scary. Poor girl's lived a life of defeat.

Kaguya.

The Lunar Capital attempted to execute her A LOT before sending her to Earth.

She could've been killed in a variety of painful ways, and she would've been completely conscious throughout the process.

And she was completely alone.
