VN projects usually fail because people don't know what they're getting into. They don't realize that you actually have to do work--consistent work--to create the finished product. You need one very devoted artist (the workload is monumental for 1 person: don't forget backgrounds!) or two that are will to collaborate styles to some extent.
You need either a few writers who are willing to put out heaps and heaps of material in regular intervals and editors who are able to keep up with them. Not only that, you need competent writers. Writers who aren't just 'trying this writing thing out' because then your editors will end up spending all of their time fixing newbie grammar/flow/structure problems in addition to trying to think critically what they're reading. Even that becomes almost impossible because if the editor doesn't have faith in the writer to create a coherent, thought-out story, then he's going to be second-guessing the writer every step of the way.

People can say, "No, I understand how much work it is," and then they expect, somehow, in a year or 10 months or something that the project will just be done. Months go by without regular work progress and everyone's expectation is that the finish is still in that vague distance, not realizing they haven't even gotten started.
They have a go at worldbuilding, story structure, characters and think "This isn't so bad!" but that's the EASY part. Anyone can think about a story. The execution is everything.

Sorry if I seem bitter... I've been a part of a few VN projects myself and they all end for the same reasons. People say they know how much work it is, but then never do the actual work. Sure, things like a crappy leader or stubborn team members can contribute to the failure, but it all comes down to whether the work gets done or not.
