I'm a sadist... to me it is a combination of things.
For one it feels like an extreme form of domination. The fact that I can make them scream and beg for their life and for me not to do it turns me on. I like thinking about their screaming as their blood pools. And like another anon said, the shock factor, but for them: can't believe it is happening, terrified, totally under my control.
And I've had a thing for amputee for a while. Missing limbs was simply appealing, if I had to take a quick guess it would be something about the proportions of the body to the limbs. Only without guro at first, but now adding guro just amplifies it.
That said, I like guro with more body deformation. Stuff that just hurts terribly without too notable of a physical impact I don't like too much.
It also has appealing parts that are non-domination. I think it is just the extreme factor in doing it, slicing open a hot girl's tummy for enjoyment, seeing their insides (especially intestines) everywhere; slicing off their arms for pleasure, I want to see them squirm in pain.
And honestly, I would have been the last person to have thought I would be into guro.
