Sanae doesn't really count because she is an infant Goddess herself according to Kanako. Kanako and Suwako moved to Gensokyo with Sanae so that Sanae can become the God she was born to be, but had no chance of becoming in the irreligious outside world.
And if she wasn't a Goddess herself, if she is Suwako's Miko, Suwako is a fertility god, her Miko probably are encouraged to have sex and produce offspring.
Until the 13th Century the most prestigious Miko in Japan was the Miko of Amaterasu at the Ise Shrine, who also had to be a daughter of the current Emperor. She was most commonly refered to as just the 'Ise Virgin' The position was eliminated when a sitting Miko was caught having an affair with the Priest of the Shrine. Under pressure from the Shogun the Emperor transferred the Miko's duties to the Priests.

Family is a pretty big thing in rural settings. That Marisa has one, but voluntarily chooses to live in the Magical Forest at a young age serves to depict her as an extraordinary, even abnormal character. But her family is there, they're mentioned.
Reimu's family is never mentioned, although they should be the most prominent, well-known and respected people around. It stands to reason that the family that provides the shrine maiden or priest is also tasked with maintaining and directly supporting the shrine, which is a rather important job. Even if that was more of a community-based thing, family members usually stick together and care for each other. What we do know is that Reimu lives at the shrine alone, the only other human that visits is some other loner that lives out in the woods, and that she gets close to zero support from humans. Why would her family, with the important duty of maintaining the balance between humans and youkai, not support her?

No children or elderly parents to take care of.
