Consider the following. What if Okuu and Orin never met and became friends? Orin wouldn't have sent the spirits out from the geyser and lured Reimu into Old Hell, and Reimu never would have bothered to investigate the cause of them since they provided them with hot springs. Eventually, Okuu's power would have continued to spiral out of control leading to Gensokyos inevitable doom via nuclear apocalypse likely killing everyone and possibly even herself. Orin saved her best friend and all of Gensokyo! With some help, of course. Good job Orin.

I believe Okuu works alone at the reactor so it would take awhile for anyone to have noticed something was wrong with her. The only one that she has any contact with there besides spirits and other random hell ravens is Orin when she delivers the corpses.
