The Touhou fanbase is large in Japan. A lot of fanbases are. It's still niche, though.

I think people misinterpret the size simply because of the sheer massive fucking output of material for Touhou.

I'm a c++ n00b and to be honest with you I see myself surpassing ZUN in 2 more years. I can already draw better than him (not that that's hard). The thing that separates a good coder from a bad coder is variety. ZUN only knows how to code bullet shooters. I'm a C++ n00b yet I can already picture myself coding games like Metal Gear and Half Life in a few years. ZUN has been coding for 15 years now and he's still doing that primitive crap. If you look at ZUN's progress through all of these years you'll notice that he hasn't evolved much. Now don't get me wrong, I fap all day and only practice coding every 2 weeks. Yet I still can see myself surpassing ZUN by the end of this year. Also, Curtain shooters are very easy to code.
