I'm not sure I would like that. Okuu and Orin have their whole best friend thing going on in the family. They might as well be the other pair of sisters. Having a third would mess that up. Maybe if it was a cute loli dog girl...

She'd be a perfect candidate for a character like Charade from Soul Calibur who would randomly select a character's abilities to copy each round. Perhaps she can also have a move that lets her change to a different character's moveset as well.

I could see Okuu freaking out and raiding the surface world because she thought Satori-sama was captured by someone after she noticed her missing. But it turns out she was just having a tea party with Reimu, and she actually told Okuu that but she forgot.

Hijiri has a strong interest in Koishi and wants her to join her side.
Chireiden will come to the surface to stop Hijiri from exploiting her.
However-will they get exploited themselves? By Kanako perhaps? Who from her past attempts at exploitation of them knows them better than most other surface dwellers, and offers to help them get Koishi back if the help her in the religous struggle with Byakuren and Toyosatomimi...
In the end though they are defeated by Reimu and Marisa, and after Marisa steals Koishi from the Temple, Chireiden pledges loyalty to the Hakurei Shrine but Reimu only asks that they go home.
They will return though for the final climactic battle of their own volition to support the Hakurei-gumi

A "Side B" Scenerio would be just the opposite, Hijiri befriends Koishi, and Satori comes to the surface and is exploited by Hijiri- because of Kanako's past exploitation. Further it turns out that Hijiri is such a complete hypocrite that Satori cannot read Hijiri's intent in her thougths because Hijiri so throughly beleves her own propaganda is truth!

Ends the same way though, beaten by Reimu and Marisa, Marisa steals back Koishi, Reimu only asks that they go home and live peacefully. They voluntarily return at the end to fight on Reimu's behalf, or at least send Okuu.
