We've been over this some months ago. If you bother to do a reverse research with TinEye you'll find out that the answer is on the eroge publisher's page.
Basically the man on left is lifting the girl's breasts by her nipples and she's forced to suck his cock, which is between her tits. Since the artist who drew it is an amateur with no real knowledge of human anatomy, her head is unnaturally bent 90 degrees forward, making it look as if it's detached from her body.
