The problem is the west likes different stuff than Japan does. So even with the small amount of art, it's not to our taste.

The west likes thick disposables mainly like >>13214125 (one of the few Japanese artists who does as well) while Japan mostly cares for its thin cloth diapers.

And the art output is low...really low. I can count the amount of artists on Pixiv who make good art for this fetish on my goddamn hand and they all take weeks or months to make something new. It truly is suffering.

Like most of you cannot even begin to comprehend how little Japan cares for this fetish. /d/iaper threads are 90% western art because of this.
