I think it's reasonable to believe that vampires like Remi and Flan are exceptions to this.
In Remi's official EoSD profile there's this paragraph:
>She is a light eater. In other words, she can drink only a little blood at a time and must leave the rest behind, so the humans off of whom she feeds become a bit anemic, but usually stay alive. Due to that fact, she always fails to increase the number of her kind.
"fails to increase the number of her kind because she doesn't drink enougn blood". To me this suggests that vampires in Touhou do procreate by draining humans of their blood, like they do in most other stories. It's possible that Remi and Flan might have had real human parents and also a "vampire parent".
