There is no explanation, it's part of the "bizarre" theme of the route. I think it exists to make you question reality, if Kaoru can see his illusions doesn't that mean he's not real too? How much of what is show in the end is real? Kaoru definitely has some sort of ethereal existence to him, almost like if he's a youkai.
Even in the SS Tama wonder if Kaoru is actually real. (Of course with the extra material we know he is real). Hashihime's last route is kinda of a flop in term of themes, both it and Hanazawa's route felt like the author has certain ideas but she she couldn't quite put into the work. At least Hanazawa's SS fixes some issue but the route is wash.

Also remember both Tama and the Shopkeeper can trap other people in their illusions, route 1,2 and 5 both have illusions battles.
