It's kind of random, essentially they select packages that come from outside of Europe and if they think the contents are worth more than 12€ they'll try to extort money out of you or force you to pay fines if the items are controlled or banned. Of course, this doesn't always happen since some items are deceptively more expensive than their packaging leads officers to believe but for the most part it's a bullshit process to deter people from daring to import things during the age of globalism.
Having just dealt with customs today I have to say that it's a bit of a pain in the ass over here mostly because the process will take more than one day. Another thing to note is that there's no "Sex Toy" category for the customs declaration so it's very likely that you might have to pay extra in customs just because it's a "Toys/Other" sort of item.
Definitely keep the extra costs in mind when ordering from abroad, you shouldn't assume that your package will get lucky much less if there's obscenity laws that could fine you for ordering something with 2lewd5me packaging.

Also, reminder: If you're ordering from within Europe and the shipping tracker says that it's being shipped from a country outside of Europe (like Switzerland or the UK) complain to the company since you're the one that'll have to pay customs on top of shipping if it wasn't free.
