1. They have foam padding inside. Some might have a strap around the back to help hold it on, but usually something adjustable, not elastic.

2. The one-part mask has the advantage of being easier to put on and be wearable by a wider variety of head sizes.

3. I'm not sure what you mean here, something like this picture? I think it's usually hidden foam though.
