We definitely have something fishy going on with the hospitals. This whole sequence basically confirms it. He was somewhere else initially and Saiki(?), Akiha and Arach(?) visited him over there.
Also, unrelated to the sequence above, Shiki immediately changes rooms at the start of the game. It just looks like a standard room switch after surgery but it's pretty jarring since the change is literally never mentioned despite Shiki saying he couldn't move from the bed for a couple of days.
