Maybe you're being sarcastic, but if you're serious, you really need to read Cage in Lunatic Runagate, which is really blatant in showing just how sociopathic Eirin really is, and it has several instances of her starting with murder as a solution to her problems, even if she ultimately doesn't go through with it. It's just the kind of person she is.
Even the whole "doctor" thing is just her following a social contract now that it's decided that Eientei will no longer completely isolate itself. It's a completely calculated move to fit in better with the rest of Gensokyo, rather than something she does out of the goodness of her heart.

Everything Eirin does is calculated and executed without feeling or remorse. She's fucking terrifying, even if she would probably act very sweetly to you. But make no mistake, it's all done just so you won't notice the gilded bars on your cage.

While interesting (the idea that Eirin was ready and willing to fucking murder Patchouli depending on how she answered her questions), I can't help but feel this might have been another one of those artist fuckups that SSiB is apparently littered with.
