In canon, Alice doesn't have a sentient doll but she's striving to make one.

In fan works, Alice talks to her Shanghai doll like it has a mind of its own. By ignoring the fact above, they are unwittingly making her out to be some sort of woman driven into insanity by years of solitude. Her obsession with Marisa in these doujins must have come from Marisa visiting from time to time to take care of Alice's chores in her stead, because Alice has slowly been unable to function because of said insanity. In her times of lucidity she realizes that Marisa had been taking care of her, manifesting as confused romantic attraction from time to time.
