It's "draw and/or describe your view of Yuuka-san," a fad targeting artists who are fans of this lovely creature. I personally haven't got around to doing it yet, although I've translated the prompts for future convenience. For clarification, the lowest one means that you can organize the fields however you want—like Kamakura-senpai did, moving her "hobbies and preferences" to the lower-left field. I interpreted the "space" as pertaining to that particular field originally, but it seems it is related to the whole char sheet.
