Different anon chiming in here, I think the reason you can get away with incest is because a writer can come up with a variety of halfway believable reasons why two blood-related people would want to fuck without particularly needing to warp their personalities too much. It also helps the reader is usually willing to meet them halfway in terms of suspending disbelief.

For harems, you either have to lobotomize a big chunk of what we expect a normal (or normal for 2D at least) person to act like, or else come up with some truly extravagant circumstances in order to make it plausible (see Harukuru for a good example). Or else the reader is willing to meet them all the way over on the other side and don't really care about a sense of realism (see harems in typical nukige).
