Before The Dark Knight, unless you were a real Batman fan, people in general had a problem connecting with or accepting the villains. Why? Because they were just cartoony nuts out to do evil for evil's sake. The reason why the Joker and Two Face are so perfect in TDK is because you understand their motivations. They view the world differently from Bruce Wayne and for good reasons, and their worlds clash as those causes come to a head.
Likewise, a good tsundere shouldn't be insane for the sake of appealing to the fetishists. There needs to be a darn good reason why she despises (or is at least extremely reluctant towards) the idea of being publicly friendly with the object of her affection, but there also needs to be a good reason why she truly loves said person. The closest example I can get to that would be Chidori from FMP. Sagara is socially inept in the extreme, and it makes sense that she, as a high school girl, would not want to be associated too strongly with someone whose actions are so alien to herself and others. But he does save her life on a number of occasions, and despite his ineptitude, he can be relied on when it counts. The tsundere-ness of her tsundere role is justified. It's not just "I'm so emotionally childish, I cannot express affection" bullshit.
