The real "culprit" is already Hinamizawa Syndrome all over again, except with little to no build-up this time. If you were expecting anything clever or original from these manga, you must be really optimistic.
Kisaku isn't going to be any worse than Canopus; at worst his attitude will be treated as questionable and led to the situation escalating eventually, but he won't be doing anything actually evil that can't be blamed on a convenient disease or complete outsider.

Plus, the entire epilogue is framed as a video report made for TV, where the author talks about how the old Hinamizawa has been lost forever after the slaughter. That wouldn't fit Kisaku at all, but obviously suits Marutake perfectly.
