You don't have to do the extra img2img step, just tick hi-res fix and it is done by default. It first generates a low-res image and then upscales it with whatever upscaler you choose.
I generally don't bother fixing any issues in my gens, just generate hundreds of pictures and usually a few of them will be decent. Quantity over quality.
