You are worrying about something which literally doesn't matter. Make something up. Transliterate your name. Copy some cool person's name from the internet. They don't know, or care, that it's not your name. The kana is really only to provide form/pronunciation for kanji names - and is one of the many reasons why kanji is dumb.
Also, it says katakana but the form only accepts hiragana in those fields.
