The girl on OP's pic has a mandatory H-scene, in addition to an optional one.

The nurse only has one H-scene, and like most of the H-scenes, in order to unlock it you must trigger *every* flag for that particular heroine prior to reaching the event point and there's no way of knowing if you missed a flag other than reaching the event point and hoping the H-scene triggers and there's no way of knowing which day/time/location has the flag outside of trial and error.
