I use a combination of both. For example, the red groups are clone-stamped, but the blue groups were drawn by hand (and then repeated). Rotating and flipping elements around is also useful to hide the fact that they're re-used, too.

On the other hand, sometimes the source image has a regular pattern that I just need to repeat, which is the simplest kind of job. The original image here was only the right hand side with Patchouli and a few notes, but it wasn't much work to replicate that out to fill a whole wallpaper.
