I dunno. I know this stuff happens, but that girl's relationship with the protag was always pretty equal, which means any of them being the "aggressor" works. I wouldn't analyze this too much.
Completely aside of those just being random extra scenes. Which the random absolutely nonsensical threesome should tell you. It's not in the normal story for a reason. It's just an official fanfiction or whatever.

Senren Banka for example had Mako constantly apologize for being "lewd". I mean, what the fuck? She wasn't even particularly weird or lewd. But that's one fetish that exists I suppose.
