I think, because the artists are Touhou doujinka too, they just draw characters they like and don't bother to add random people/youkai .

I don't know about you but going to a festival at Reimus shrine together is pretty damn close to family stuff. That or friends.

Tenshi is probably living in heaven and visits Gensokyo from time to time.
