Of course it is, that's what Ryuukishi intended. It's not just your typical paranoia, it's literally a bunch of microscopic bugs physically messing with his brain chemicals because they got upset over the climate or whatever, which plungs him into a nighmarish, unbearable, terrifying persecution complex that gets worse by the minute.

Of course something like that would be more terrifying than some obscure shady people plotting a conspiracy. Onikakushi is meant to be the scariest part of the story, because you still know nothing about the true nature of this creepy village.
