I think touhou 13 will be a follow up on Subterranean animism, Reimu got cancer from the radiation and now gensokyo is on the brink of destruction, to save her our heroines Marisa, Sakuya and Mima will use a kappa shrink ray/yukari border manipulation/suika density manipulation to venture inside Reimus body to fight her immune system (stage 1-3) then the actual cancer etc. (stage 4-5) but then after the challeningstahe 5 boss the real mastermind is revealed. Or something along those lines.
