One explanation I've heard is that Tomosane actually died and Hasaki sees both of them. Though, ultimately, I feel, it doesn't really matter. It doesn't even matter if everything is just a delusion of a schizoid Ayana lying in the hospital that was seen in gondola ride or if it's a dying dream of Yuki or if it's all actually real or whatever. It carries that MGS2-like theme of "it doesn't matter if it's fiction and not real, what's important is what you've found for yourself inside this work."
