Gusset is a thicker, reinforced piece of fabric inserted into pantyhose crotch seam to provide additional strength and breathability (as it's often made from more breathable materials like cotton, rather than nylon).

It has been my observation that pantyhose from japan, both in 2D and 3D are often lacking this feature. The pic here is a rare exception.
