Fading Hearts Parallel. It's worth playing because it's so bad it's hilarious.

If we count only Japanese VNs, probably Panda Samurai. It simply sucked.

If we only count commercial Japanese VNs, I'd have to agree with >>7974272 on Cross Channel. It's basically all about the protagonist acting like a complete ass to others (which is supposedly comedic, but it only made me want to kill the protagonist) and nothing else happening. I couldn't bring myself to finish it.
