Japan doesn't have freedom of speech or fair use. Don't forget that they can't even draw a sketch of a penis without having to censor it.
In fact, it's even worse than you think. The government body responsible for policing artwork online is called the Internet Hotline Center and it's run by a web consultation company with no direct judicial oversight called SIEMPLE INC.

They interpret Japan's constitution, decide what is considered "obscene", and then they decide which websites need to comply with their extra-judicial interpretation of the law. Their rules aren't written down and they frequently pressure companies like Pixiv, Fantia, DMM, and DLsite to enact heavier censorship of user-submitted works. They aren't consistent, and instead rely on intimidation to force websites to self-censor. This often results in their own rules for one website being completely different from others and it's why different websites have different censorship requirements. As you can imagine, this also creates a ton of opportunity for corruption, with legal threats and prosecution for websites that refuse to comply.

Recently, several artists' works were removed from Fanza because the IHC has recently decided that the depiction of pubic hair is obscene. This particular kind of regulation hasn't been seen in decades.

Here is one such artist that announced that he is changing his current work and that going forward, the MILFs in his ero manga will all be shaved. This is in response to his other works recently being removed from websites due to the depiction of pubic hair.

Say goodbye to Mio doujins or artwork where BGM is sporting a Bushy Generous Manko. It's ridiculous and insane how backwards Japan is.
