There's always Akaneiro no somaru saka.

It feels like they're in a completely alternate dimension though, since no one bats an eye at how the main character is courting his sister in plain view.

Clover Point is the opposite of this and will make you scream in fear/delight when the identity of the sister is revealed.

I finished the Minato route recently.

Yes, yes she is blood related. Main character goes as retardedly far as saying like "Let's get married when I graduate!"...right in front of their parents and friends and so on. And they just don't seem to mind this at all.

Akaneiro must exist in some completely alternate dimension where things like that are ok. Usually games with blood related sister characters do have some kind of conflict centered around this fact as it is a sweet source of DRAMA.
