Okay so I just used my first onahole and a hole tore in the side while I was using it, did I do something wrong or bad luck?

I got interested, lurked the last thread, read the guide, measured myself and shopped around trying not to spend too much since I wasn't sure I'd love it, ended up ordering two for safety:
