It's well known that Reimu hit rock bottom and is basically a destitute husk, and since she—probably—hasn't left the shrine for a while it's probable that people think she either starved to death or is in such a sorry state she can't show her face in public anymore. The joke there is that she got desperate and became a prostitute to make ends meet. Only a very small few knows she made a 180 and is going to be one of many surprises revealed at once. Also there was probably supposed to be a comma there to give a pause like: "Reimu is what, giving handies behind the shrine to make money?"
