Keiichi convinced himself that he disappeared from the world on the night of Watanagashi, as the disappearance victim of the fifth year of the curse of Oyashiro-sama, and that he ended up in a nightmare world to torture him.
He wants to believe that Satoko is happy in the supposed real world without him, so he decides she must be smiling there, but he never actually saw any of that.
Satoko only wanted Keiichi to die and would never smile for him.
