I wouldn't treat Lost Word as canon for two reasons:
1. ZUN likely does not do any writing input for it. If he did, their marketing would undoubtedly flaunt that.
2. LW frames itself as an incident within a multiverse. What's canon in the primary LW-verse does not necessarily reflect what's canon for Touhou proper.
Koa being tied to Patchouli is the most common and straightforward interpretation but it hasn't been explicitly canonized to my knowledge.
