Dollfie Dream bodies have softer vinyl parts sitting on top of an internal skeleton. "sculpting" the chest will most likely result in cutting a hole straight through to the internal skeleton.

Alternatively, you could shape the bust you want, turn it into a mold, and use silicon to fill the mold. Then you just need to cut holes in the torso for the joins to fit into the internal skeleton.

As for paint, you just need to find paint that is safe for vinyl, and it shouldn't cause bubbling or warping of the body.
