Very seldom. The problem with vanilla is that it's just well, vanilla that goes through roughly the same hoops every single time. Harem usually doesn't do it for me either as it's just girls taking turns to service the protagonist, instead of pleasuring each other as well. I'm not saying they should add notably darker sexual content, just that H-scenes seem to lack the actual erotic part and the feeling that both partners are really indulging in their carnal desires.

One notable good exception I found was Orietta's last H-scene in MagiCha. Not really a spoiler, but tagged just in case people want to experience the naughty first hand. What she does is she secretly purchases hardcore porn videos behind the protagonist's back so she could learn to have better sex with him. The protagonist learn about this and after pondering they decide to watch them together, she gets really horny while watching and starts masturbating unconsciously. Next they'll proceed to have rough sex with her coming multiple times while crying out REALLY loud.. Her VA in that scene was 10/10 to the point it wouldn't be surprising if they opted to draw her with fucking ahegao (they didn't). I think a "safe" way to make vanilla sex scenes much better would be if the VAs actually did it like they meaned it akin to more hardcore works, without actually adding hardcore sexual content.

To give a more concrete non-eroge example, I think there was this one ShindoL doujin with a handicapped girl which was completely vanilla relationships wise, but the girl was going absolutely crazy while having sex. Stuff like that would be welcome.

Surely you jest. Unless you're talking about console VNs even most plotge are eroge.

Me neither. Most plotge and moege don't really manage to make the sex scenes actually erotic. I think the last non-nukige with somewhat enjoyable H-scenes that I played was MagiCha's Orietta route.
