There has been multiple threads around 4chan about how everyone in the past was flat chested and small due to the lack of nutrition, and large breasts are a modern thing as well.
ZUN also tends to often draw Sanae with bigger breasts than Reimu's, therefore I'm pretty sure he even acknowledges that too.

I would love to see a small and flat Sanae that didn't developed with contemporary nutrition, though.
