I could be wrong but to my mind its similar to Takano face death by lightening in Higurashi. Lambda represents controlling destiny by sheer force of will so neither Sayo or Takano has to meet a literal physical representation of her to be under her protection, because she is a sort of universal concept like most gods/witches.

So basically how literal you interpret Lambda's encounters is up to you. I think it happens to anyone that uses the 'magic' of certain willpower to achieve their desired result.
