Since Mugetsu created the dream world i think it would be easy for her to change her form to whatever she wants (at least inside her world). That would mean that her boobs are real and not a fake.
Gengetsu probably has this power too, that could explain why she has wings while Mugetsu doesn't despite being her sister.
It would also mean that Genge isnt some jealous flattie but deliberately chose her form either for aesthetic or aerodynamic reasons
