It's the narration of the Extra Stage that brings up the belief that eating mermaid flesh would grant immortality, and it's likewise only the protagonists who talk about how eating an immortal's liver would bestow immortality on the consumer, with all of them having their own ideas about how it works, and neither Mokou, nor the two actual authorities on the subject, Eirin and Kaguya, ever confirm that it's true. It's quite likely that the protagonists simply make assumptions based on other myths about immortality and assume the same applies to the elixir, but as the Hourai Elixir is legendary and obscure even amongst such legends, their judgement is questionable.

On a similar note, when Mokou talks about needing to take three sips from the elixir, there's nothing that really supports it, as there's no instance of someone only taking a partial dose. Mokou was probably just trying to be poetic there about how consuming the elixir eventually only leads to despair.
