It's not confirmation bias. The game knows what you want. It teases you with crumbs of success, only to crush your hopes moments later.

Like that time I was amazed at how well the 2-3 runs were going - everything perfect - but had in fact forgotten to activate the quests.

These days I've learned my lessons - I activate every sortie quest whenever I sortie, even if I'm just grinding 3-2.
Just in case.

Of course, this means nothing ever goes perfectly - but such sacrifices are necessary.
