My first thought was Eirin, but she's a pharmacist, not a surgeon.
For all we know, she could be shit at surgery. She's pretty smart though and good with medical practices though, so I doubt it. She's probably pretty good.

But I'd say Yumeko. She already uses full swords as "throwing knives" and danmaku, so she should be pretty skilled with using blades too large for whatever activity is at hand.
