So, a followup. Looked through the code and it appears to be a storyline-exclusive trait. The following girls can get it:

If you have barren sex for long enough (after 200 V EXP and not being pregnant)

One of her love dialogues is "I want to have 70 children", so not surprising that some event triggers after getting love+lover
>Hatate
Shouldn't really be a surprise since her game file is like three times the size of the largest character translated so far. She probably has flags and events for every possible scenario imaginable.
>Eiki
Again, not surprising; the rigid, inflexible religious judge wants sex purely for procreation
>Eika
The stillborn spirit. Yeah, this one seems weird but it's referenced in the code
>Flandere
Requires a bunch of weird triggers including one pain mark, one abnormal mark, 10000 love xp, 100 date xp, no pregnancy before
