I'm just going by my own headcanon, you could be right. They can indeed use a real person's face. I just think they keep the full-on realism and details reserved for under-the-table personal commissions. As real as the default dolls they sell online look, you can still tell they're dolls, so I imagine they take some creative liberties and tweak some stuff so they don't trigger anyone.
