The attitude Misumaru has towards Reimu throughout the dialogue they share in her boss encounter and in the alternate ending of Reimu all kind of set up Misumaru for a grandma role, granted it takes the usual exaggerations and loose nature of the fandom.
She's protective of Reimu to a degree since she's using the Yin Yang Orbs, really not wanting Reimu to die since Stage 4 is an anoxic mine. She also provides a magatama needed to explore it in the Extra stage (Granted that's for every character. She's on your side and is the one offering you a card before fighting Momoyo, and laments the fact you're fighting Momoyo, which is essentially Tsukasa's fault). She's proud of Reimu once she's defeated, and when invited back to the shrine, she explains a bunch of stuff which Reimu finds boring in the same way a child could find the stories of their grandparents at a dinner table boring.
